Registered Nurses do all the in home assessments with the client. Family members may be present during the admission assessment and together a care plan to meet specific needs and wishes is created. The Registered Nurse, Certified Nursing Assistant and Homemaker programs all use job specific care plans to deliver consistent and thorough care with Registered Nurse supervision.

The Registered Nurse Case Management Program provides a Registered Nurse to coordinate the care for the client. It is as involved as the client needs. For example, it can be total management of the client’s activities of daily living, making appointments, attending appointments and follow-up with the home care needs. In some cases the nurse would report to the designated family member who oversees the client care but who may, for example, live in another state. With this program, the Registered Nurse works with the client/family to coordinate a plan around needs and wishes. The client can be maintained safely in the home and surroundings. Often with this program the Homemaker services and Certified Nursing Services are utilized along with the Registered Nurse’s coordination and the cost can become more reasonable than some alternatives. Registered Nurse visits can also be made for clients needing just a medication pre-fill. Teaching and education can be done as well as vital sign checks and wound care visits. They are charged at a visit rate.
